(b) description of items for sale, (c) estimated cost of items for sale, (d) sales price of items,
(e) description and estimated cost of related fundraiser expenses, (f) description of method of sales
and collections, and (g) estimated profit from fundraiser. A separate sheet may be attached to this form
if more space is needed.

I certify that this fundraising event will comply with all Board of Education and local school guidelines.
These include but are not limited to:
1. Fundraising will not involve the selling of items door-to-door.
2. All purchases will be preceded by an authorized purchase order.
3. Money will be receipted by the teacher and submitted to the office daily.
4. All fundraising expenses, including sales awards to students, will be presented
to the Principal for payment by check.
5. A school employee will not receive a gift or gratuity from the fundraising vendor.
6. A retail accountability report will be completed by the organization's sponsor.
7. Tickets will be used for any fundraiser that charges admission.
